Important Appointment Information
To help us provide the best possible care, 1) please arrive a few minutes early for your appointment, 2) bring any previous medical records or medication information if applicable, and 3) complete any required online forms ahead of time when possible.
New Clients
For new clients, we encourage you to complete our New Client Form before your visit. You may also wish to visit our New Client Center to learn what to expect at your appointment.
Cancellation Policy
Effective April 21, 2025, all appointments are subject to our cancellation policy.
If you do not show for your appointment or fail to provide timely cancellation notice, a $28 fee may be added to your account. To avoid this fee, please notify us of cancellation at least 12 hours before your scheduled appointment time.
A $28 deposit is required for all new client appointments.
